Converter substrates do not 'break loose or start rattling' Their shell is welded inside the body. If the substrate core actually breaks apart, this would mean it was overheated by an engine that is ill-running and burning the wrong air/fuel mixture and thus mealted the substrate to the extreme of breaking it apart. This would not be the fault of the converter but the engine. This will be determined if the converter is cut opene and the substrate examined.
